 Welcome to our
new web based form for the 2007 Annual Shellfish
Management Review. We anticipate that those who are
comfortable with web based forms will find it more
convenient than the standard review form. This form
was designed with first time users in mind,
however, and all are encouraged to give it a
try.
The instructions for filling out this
form are basically the same as for the paper form
mailed to you so be sure to review those
instructions before you begin. Please note: it is
imperative that each page of this form be filled
out completely and submitted before moving on to
the next to avoid accidental loss of the
data.
Once a page is filled out, it is
submitted to DMR via e-mail by clicking on the
submit button at the bottom of the page. A message
will pop up indicating that the process was
successful and suggesting that you print the
submitted page. Use the "Continue" button at the
bottom of each page to advance through the form.
This form does not have to be
completed in one sitting. Complete and submit as
many pages as you wish. To resume where you left
off at a later date, simply access this site again
and proceed through the form by using the
"Continue" button at the bottom of each
page.
Once all of the pages have been
submitted, you may return to any page by using your
browser's back button. Should any corrections be
needed to any page, simply make the changes and
resubmit the page. However, this can only be done
in a decending order if more than one page needs to
be corrected (i.e., Page 12 first, Page 1
last).
